A bstract Eyelid eversion is a simple yet frequently neglected component of routine ophthalmic examination. Despite its simplicity, it is invaluable for detecting clinically significant findings that may otherwise remain unnoticed. Subtarsal foreign bodies, concretions, papillary or follicular reactions, cysts, tumors, and early ocular surface disorders are often identified only after proper lid eversion. Patients presenting with watering, recurrent redness, foreign body sensation, or non-resolving superficial keratitis may harbor subtle upper tarsal pathology that can be missed if this step is omitted. Failure to perform eyelid eversion may lead to misdiagnosis, inappropriate treatment, persistent symptoms, and preventable corneal complications. This report emphasizes the importance of incorporating eyelid eversion as a routine and indispensable part of comprehensive anterior segment evaluation.
