Case report highlights phaco tip fracture risks in cataract surgery, suggesting improved management strategies.
A bstract Phacoemulsification is the gold standard in cataract surgery, and the integrity of the phacoemulsification tip is critical for successful outcomes. We report a rare intraoperative fracture of the phaco tip during cataract surgery in a 74-year-old male with Grade 4 nuclear sclerosis. The break occurred at the distal end within the sleeve, leading to sudden aspiration loss. The anterior chamber was stabilized, the fractured tip removed, and surgery completed with a new tip. Postoperative recovery was uneventful. The article reviews literature on the causes of phaco tip fracture, including manufacturing defects, metal fatigue, instrumental trauma, and excessive ultrasound energy. Clinical implications, intraoperative management strategies, and preventive measures are discussed. Vigilant inspection and standardized protocols are crucial to avoid such complications.
