Metaplastic breast carcinoma (MpBC) is a rare and aggressive histological subtype of breast cancer, defined as a mixture of adenocarcinoma with mesenchymal and epithelial components. The management of these patients is challenging due to the rarity and heterogeneity of the disease. We present the case of a 46-year-old woman with a 1.5 cm right breast tumor, treated with conservative breast surgery and sentinel lymph node biopsy. Pathological examination revealed invasive ductal carcinoma with sarcoma-like structures, indistinct differentiation, and cartilage-like morphology. The patient underwent adjuvant chemotherapy with docetaxel followed by epirubicin and cyclophosphamide, adjuvant radiotherapy, and endocrine therapy. At the 2-year follow-up, no recurrent or metastatic lesions were observed.
