Key Points

  • To evaluate biochemical signatures in cerebrospinal fluid to differentiate between leptomeningeal carcinomatosis and infectious meningitis.
  • Retrospective cohort analysis conducted at a single center.
  • Biochemical analysis of cerebrospinal fluid samples was performed.
  • Diagnostic performance of lactate and glucose levels was assessed.
  • High CSF lactate levels indicate bacterial infection or aggressive tumors.
  • Normal glucose and low lactate levels are associated with viral infections.
  • Distinct CSF signatures identified between neoplastic and infectious meningitis.

Abstract

These findings highlight that certain biochemical signatures can distinguish etiologies whereby high CSF lactate favors bacterial infection or aggressive tumor involvement, whereas normal glucose and low lactate favor viral causes. Our results emphasize distinct CSF signatures in neoplastic compared to infectious meningitis, suggesting metabolic profiling can aid diagnosis.
