Purpose To compare the long‐term positive dysphotopsia (PD) rates associated with various intraocular lenses (IOLs) relative to the rates of PD associated with the Mx60E IOLs. Materials and Methods This was a retrospective cohort study design without randomization. Patients were selected by cross referencing lists of IOL implantations and ophthalmology clinic visits logged in the institution’s electronic medical record. Exclusion criteria included comorbid glaucoma, comorbid retinal disease, posterior capsular opacification, lack of follow up > 6 months post IOL implantation, and visual acuity worse than 20/40 in the eye of interest. PD outcomes were determined by either confirmed diagnosis included in the patient’s chart > 6 months after IOL implantation or positive score on Pseudophakic Dysphotopsia Questionnaire 6. Results A total of 208 eyes were included in the final analysis, of which 129 (62.02%) were female and 79 (37.98%) were male. Odds ratios for PD development of the AU00T0, PCB00/ZCB00, and SN60WF lenses relative to the Mx60E lens were 14.40 ( p = 0.0146), 11.56 ( p = 0.0217), and 8.31 ( p = 0.043), respectively, all of which satisfied the statistical significance criteria of p < 0.05. Conclusions The Mx60E lens had significantly lower rates of long‐term PD in patients relative to the AU00T0, PCB00/ZCB00, and SN60WF lenses. These results suggest that PD may be an additional consideration in the final determination of which implanted IOL to use in a given patient.
