Case study documents governance processes and findings in a three-agent healthcare workflow.
This case study documents the complete governance chain for a new healthcare SMB client — TENANT_CEDAR_HEALTH — onboarding onto the APR-Series governance stack with a three-agent sequential workflow. The workflow consists of Patient Intake Agent, Clinical Summary Agent, and Compliance Review Agent, operating sequentially to receive, summarize, and verify clinical documentation. The case study walks seven acts: formation governance (APR-Shaper), governance identity issuance and pre-registration, runtime substrate configuration (APR-Lite), enforcement at decision time (APR-Guard), evidence and chain verification, integrity verification, and a full-stack summary with governance findings. All artifacts are real: governance_profile_ids, formation scores, receipt IDs, chain tips, manifest hashes, and integrity hashes are drawn from live system state as of April 6, 2026. No data has been fabricated. Two governance findings surfaced during the live run that are documented in Act IV.
