This article presents the implementation of a structured tier-based orientation model at a midwestern academic healthcare system. Using the Plan-Do-Study-Act QI framework, the model was piloted on two units and expanded across the organization over 4 years. Post implementation data revealed that 97% of orientees and 84% of preceptors felt supported throughout orientation, 89% of orientees reported structured orientation, and 99% reported confidence to provide safe care. Organizational RN turnover rate decreased from 7% to 3%.
