Abstract

背側膵動脈瘤は極めて稀な疾患であり,腹腔動脈狭窄・閉塞と関連している。症例は76歳男性,偶発的に正中弓状靭帯圧迫症候群に起因する腹腔動脈起始部閉塞と背側膵動脈瘤を発見された。膵十二指腸アーケードには高度な石灰化狭窄を散見したため直達手術にて動脈瘤空置と順行性バイパスを実施した。膵十二指腸領域の動脈瘤ではコイル塞栓が推奨されているが,血行動態に懸念が残る場合には積極的な血行再建も選択肢となりうる。
