To evaluate the effect of metformin on cancer incidence and mortality among diabetic patients compared with other antidiabetic therapies.
Systematic search of PubMed, ISI Web of Science, Embase, and the Cochrane Library through May 2009 without language or time restrictions.
Identified and independently evaluated 11 epidemiologic studies capturing 4,042 cancer events and 529 cancer deaths.
Metformin treatment was associated with a 31% reduction in overall cancer incidence compared with other antidiabetic agents (summary relative risk = 0.69; 95% CI, 0.61–0.79).
Statistically significant risk reductions were identified for pancreatic and hepatocellular cancers along with a trend toward a dose-response relationship, whereas associations for colon, breast, and prostate cancers were nonsignificant.
