CNS TB in our cohort predominantly affected young adults and presented with classical meningeal features and diverse neuroimaging abnormalities. Microbiological confirmation remained limited, underscoring reliance on clinical-radiological diagnosis. Poor outcome was associated with increasing age, longer duration of illness, cranial neuropathy, hydrocephalus, and paradoxical reaction.
