Does peri-procedural DOAC interruption affect the risk of pocket haematoma and thromboembolic events in patients undergoing cardiac electronic device implantation?
Does peri-procedural DOAC interruption affect the risk of pocket haematoma and thromboembolic events in patients undergoing cardiac electronic device implantation?
In patients undergoing cardiac device implantation, peri-procedural DOAC interruption is associated with rare clinically relevant haematomas and thromboembolic events, suggesting bleeding risk outweighs thromboembolic risk in this setting.
Supports DOAC interruption in CIED implantation with low event rates; hypothesis-generating and requires RCT confirmation before practice change.
In this large real-life registry where most patients had DOAC interruption, clinically relevant haematoma was rare. Despite DOAC interruption and high CHA2DS2-VASc score, thromboembolic events occurred seldomly, highlighting that bleeding exceeds thromboembolic risk in this peri-procedural period. Future research is needed to identify risk factors for clinically relevant haematoma and meaningfully guide clinicians in optimizing DOAC management.
