This study compares the visual outcomes, safety, and complications of two fixation techniques for intraocular lenses in aphakic eyes.
Prospective randomized controlled trial design
50 aphakic eyes from 50 patients
Allocated equally to iris claw (n=25) or iris suture (n=25) fixation
Evaluated at 1, 3, 6, and 12 months postoperatively
Measured corrected distance visual acuity (CDVA), surgical duration, IOP, and complications.
Both techniques showed significant CDVA improvement from baseline (p < 0.001)
Mean surgical duration was shorter for iris claw fixation (30.5 min) than for iris suture (46.4 min; p < 0.001)
Lower mean spherical equivalent in iris claw group (−0.30 diopters) compared to iris suture (−0.50 diopters; p = 0.08)
Complication rates were low, with one disenclavation in the iris claw group and mild erosions in the iris suture group.
Significantly greater endothelial cell loss noted in the iris claw group (14.5% vs. 9.8%; p < 0.001).
