P15 | Randomized Comparison of Isatuximab–rd Induction Followed by Isa R Maintenance vs. Rd Induction Followed by R Maintenance in Elderly Patients With Newly Diagnosed Multiple Myeloma (Agmt Mm04 Trial)
Randomized trial shows Isatuximab-RD improves outcomes in elderly patients with newly diagnosed multiple myeloma, suggesting clinical benefits.
Key Points
To evaluate the efficacy of Isatuximab-lenalidomide-dexamethasone (Isa-Rd) induction therapy followed by Isa maintenance versus lenalidomide induction and maintenance in elderly patients with newly diagnosed multiple myeloma.
Prospective, randomized trial comparing two treatment regimens
Patients received 8 cycles of either Isatuximab-RD or Lenalidomide-Dexamethasone
Maintenance therapy continued for 24 cycles
Minimal residual disease (MRD) was evaluated at the end of induction
Progression-free survival (PFS) and overall survival (OS) were analyzed using Kaplan-Meier methods
Higher complete response (CR) and very good partial response (VGPR) rates observed in Isa-Rd group (CR: 26% vs. 10%)
Significantly more patients achieved MRD negativity in Isa-Rd group (34.6% vs. 2.5%)
Median PFS was longer in Isa-Rd group (31.8 months vs. 22.2 months)
A tendency for improved overall survival in Isa-Rd group; not yet significant (median OS not reached vs. 48.1 months)
