Trauma centers report challenges in managing pelvic fractures and hemorrhagic shock, suggesting algorithmic approaches to reduce errors.
Pelvic fractures associated with hemorrhagic shock are challenging problems that are often encountered in major trauma centers. The need for sound decision-making is pressing in these situations. Taking the wrong path in management (for instance, performing laparotomy when it is not indicated) can lead to a catastrophic outcome. The fundamental question facing the trauma surgeon is where the major source of bleeding that accounts for the hemodynamic instability and represents the priority in taking the appropriate measures to stop the bleeding. Establishing an algorithmic approach will minimize the potential for critical errors in the management of these patients.
