上腕骨近位・骨幹部粉砕骨折に対しリバース型人工関節置換術を施行した1例

Abstract

上腕骨近位・骨幹部粉砕骨折にリバース型人工肩関節置換術（RSA）を施行し良好な結果が得られた1例を報告する．症例は72歳，女性．草むしり中に5 mの高さから転落し右半身を打撲する形で受傷した．結節の著明な粉砕を伴う右上腕骨近位から骨幹部の粉砕骨折であり，受傷9日目にRSAを施行した．Stryker社Aequalis Reversed Fractureのロングステムを使用し，粉砕した骨幹部の骨片をステムに巻き付けるようにして固定し，遠位部をセメント固定した．右肩は術後2週間安静とし，以後腱板断裂術後プロトコールに準じてROM訓練を開始した．術後1年の右肩の可動域は挙上140°，屈曲130°，外転140°，外旋10°，内旋L1であり，JOAスコアは86.5点，ASESスコアは85ポイントと良好であった．RSAは，結節の粉砕を伴う上腕骨近位端骨折に対して有効な手術法である．本症例では，骨幹部までの粉砕を伴う重度の骨折に対して，骨折用のロングステムを使用することで良好な結果を得ることができた．
