【症例1】84歳女性．大腿骨頚基部骨折に対して，閉鎖的に整復し髄内釘で内固定を行った．整復位は正面像解剖型，側面像髄外型であった．後方骨片は粉砕しており，術後8週でカットアウトが生じた．【症例2】91歳女性．小転子まで至る大腿骨転子部骨折に対して，直視下に整復し髄内釘で内固定を行った．術後骨頭骨片は回旋転位が残存しており，整復位は正面像外方型，側面像髄内型であった．術後2週でカットアウトが生じた．【症例3】90歳女性．小転子まで至る大腿骨転子部骨折に対して，直視下に整復し髄内釘で内固定を行った．整復位は正面像内方型，側面像髄外型であり，術後3カ月でカットアウトが生じることなく経過している．【考察】適切な手術適応と適切な整復位保持が重要であると思われた．
