Abstract Background/Aims Patient information leaflets (PILs) play a crucial role in empowering rheumatology patients who often manage complex chronic conditions and therapies with potential side effects. Traditionally paper-based, PILs enhance patient knowledge and satisfaction and may support treatment adherence. However, many rheumatology patients are younger, adept with technology, and may prefer digital formats. Electronic PILs (ePILs) offer advantages such as instant mobile access, reduced leaflet loss, accessibility for those with manual dexterity difficulties, and improved sustainability by reducing paper use. We evaluated local patient preferences and the feasibility of introducing ePILs in rheumatology clinics. Methods This single-centre service evaluation was conducted in a large district hospital in South East England. An initial survey of 50 patients attending adult rheumatology clinics in August 2024 assessed interest in ePILs and preferred access format. Based on this, we developed a prototype electronic leaflet booklet: laminated A5 cards with QR codes and URL links to Versus Arthritis resources. QR codes were generated using freely available tools. Feedback from consultants, registrars, and clinical nurse specialists informed the design. A follow-up survey of 50 patients (September-October 2024) explored users’ experiences, usability, and willingness to reuse ePILs. Results In the first survey (n = 50), 80% of patients were under age 75, and 46% had previously received PILs. Nearly half (48%) preferred ePILs to paper, with 46% favouring QR codes and 44% URL links. Only 8% reported being unable to use ePILs, with younger patients (50) showing a stronger preference for QR codes and older patients (51) for URLs. The second survey (n = 50) had similar demographics, with 75% under age 75. A majority (80%) had prior experience of QR codes, and only 12% required assistance scanning. QR codes functioned for 76% of patients, with barriers largely due to device settings or clinic Wi-Fi connectivity. Notably, although many had expressed preference for URLs in the initial survey, none used the provided links. Four patients were unable to engage due to lack of smartphone access or cybersecurity concerns. Most participants found QR codes easy to use and reported willingness to use them again. Conclusion This evaluation demonstrates strong patient acceptance of ePILs in rheumatology care, particularly QR code access. Concerns regarding digital accessibility were not confirmed, as most patients readily engaged with the technology. Introducing ePILs can streamline clinic logistics, reduce paper waste, and align with patient preferences.
