TÓM TẮT Mục tiêu: Đánh giá hiệu quả giảm đau và tính an toàn của phương pháp phong bế chọn lọc rễ thần kinh kết hợp với đại trường châm trong điều trị đau thần kinh tọa do thoát vị đĩa đệm cột sống thắt lưng. Đối tượng và phương pháp: Nghiên cứu mô tả tiến cứu, so sánh trước và sau can thiệp trên 37 bệnh nhân đau thần kinh tọa do thoát vị đĩa đệm cột sống thắt lưng thể huyết ứ. Các chỉ tiêu đánh giá bao gồm: mức độ đau theo thang điểm VAS, độ giãn cột sống, nghiệm pháp Lasègue, tầm vận động cột sống thắt lưng. Kết quả: Sau 15 ngày điều trị, các chỉ tiêu lâm sàng cải thiện rõ rệt so với trước can thiệp. Kết quả chung cho thấy 94,6% bệnh nhân đạt kết quả tốt, 5,4% đạt kết quả khá; không có trường hợp trung bình hoặc kém. Không ghi nhận tác dụng không mong muốn trong quá trình điều trị. Kết luận: Phong bế chọn lọc rễ thần kinh kết hợp với đại trường châm là phương pháp điều trị đau thần kinh tọa do thoát vị đĩa đệm cột sống thắt lưng có hiệu quả giảm đau rõ rệt và an toàn. SUMARY Objective: To evaluate the analgesic efficacy and safety of selective nerve root block combined with long needle acupuncture in the treatment of sciatica caused by lumbar disc herniation. Subjects and Methods: A prospective descriptive study with a pre–post intervention comparison was conducted on 37 patients diagnosed with sciatica due to lumbar disc herniation, classified as the blood stasis pattern according to traditional medicine. Outcome measures included pain intensity assessed by the Visual Analog Scale (VAS), lumbar spine flexibility (Schober test), Lasègue sign, and range of motion of the lumbar spine. Results: After 15 days of treatment, all clinical parameters showed significant improvement compared to baseline. Overall outcomes indicated that 94.6% of patients achieved good results, while 5.4% achieved fair results; no cases were classified as average or poor. No adverse events were recorded during the treatment period. Conclusion: The combination of selective nerve root block and long needle acupuncture is an effective and safe treatment modality for sciatica caused by lumbar disc herniation, providing significant pain relief and clinical improvement.
