Abstract Introduction Sleep disturbances are common in pregnancy and may influence both maternal health and fetal development. Nocturnal sleep duration, habitual daytime napping, and obstructive sleep apnea (OSA) are increasingly recognized as potential contributors to adverse pregnancy outcomes. However, their combined impact has not been fully characterized. Objective: To examine the associations between nocturnal sleep duration, afternoon napping habits, and OSA with key maternal and fetal outcomes during pregnancy. Methods This study investigated prospectively pregnant women receiving routine antenatal care. Nocturnal sleep duration and afternoon napping frequency were collected using actigraphy, and OSA risk was assessed using home sleep apnea testing. Maternal outcomes of interest included gestational diabetes mellitus, hypertensive disorders of pregnancy, and mode of delivery. Fetal outcomes included birthweight, preterm birth, apgar score and neonatal complications. Multivariable models were used to evaluate associations while adjusting for relevant confounders. Results Total number of participants were 430 pregnant women (Age 18-35 years). Cross-tabulation analysis indicated that presence of OSA was found to be associated with gestational diabetes (P=0.02) and short nocturnal sleep duration( 5hrs) was also found to be associated with gestational diabetes( P=0.03). There were no other significant associations between sleep parameters and maternal or fetal outcomes (P0.05) Conclusion Sleep duration and OSA each exhibit meaningful relationships with pregnancy health, mainly gestational diabetes. These findings highlight the importance of comprehensive sleep assessment in prenatal care and suggest that early identification of sleep disturbances may offer opportunities to improve both maternal and fetal outcomes.
