Background Targeted precision cryotherapy has emerged as a novel approach in aesthetic dermatology, offering enhanced precision and safety during procedures. This study evaluated the efficacy and safety of a poly‐D,L‐lactic acid–hyaluronic acid (PDLLA–HA) hybrid filler administered using a TargetCool device to improve facial skin quality. Materials and Methods A prospective controlled study was conducted in 20 Korean women with melasma, hyperpigmentation, or dark spots. One hemiface was treated with PDLLA–HA using a precision cryotherapy device, while the contralateral side received the same formulation via a microneedle roller. Skin parameters, including melanin content, dermal density, pore volume, pore density, pore count, and redness, were assessed at baseline, during treatment, and 1 month after the final session. Visual grading, patient‐reported outcomes, and safety evaluations were also performed. Results Cryotherapy‐assisted delivery demonstrated significantly greater improvements than microneedling in melanin reduction (31.28% vs. 23.40%), dermal density (30.07% vs. 17.37%), and pore‐related parameters, including volume (53.43% vs. 33.61%) and density (57.80% vs. 21.62%). Redness was significantly reduced on the cryotherapy‐treated side. Participant satisfaction was high, with consistent improvements in skin texture and overall appearance. No adverse events were observed. Conclusion Precision cryotherapy‐assisted delivery of PDLLA + HA provides superior efficacy and favorable safety compared with conventional application methods. This approach may enhance transdermal delivery while minimizing tissue trauma, representing a promising strategy for advanced aesthetic dermatologic treatments.
