42歳男性．多発性内軟骨腫と体表の血管腫からMaffucci症候群と診断されていた．経過中に内軟骨種の悪性転化を認め，肺塞栓症を合併し，抗血栓療法が開始された．その後黒色便を生じ，造影CTと血管造影で小腸出血が疑われ，カプセル内視鏡で小腸に多発する血管腫を診断した．Maffucci症候群による消化管病変は国内外で9例のみの報告であり，本症例はカプセル内視鏡で診断した貴重な症例のため報告する．
