This case report describes the successful surgical resection of a rare well-differentiated liposarcoma arising in the ileal mesentery, with no recurrence at 20 months.

症例は72歳,男性.腹痛・嘔吐にて入院した.腹部ctおよびmriで下腹部に脂肪成分に富んだ内部不均一な巨大な腫瘤を認め,上行結腸が背側に圧排されていた.小腸造影および注腸造影で腸管の圧排所見を認めたが,狭窄像は認めず.血管造影検査では淡い腫瘍濃染像を認めた.以上より,腸管外に発生した脂肪肉腫を疑い手術を行った.回腸腸間膜を基部としたダンベル型の巨大な腫瘤を認め,回腸と共に切除した.組織学的には高分化型脂肪肉腫と診断した.術後約20カ月で再発兆候は認めていない.今回,回腸腸間膜に発生したまれな脂肪肉腫の1例を経験したので文献的考察を加え報告する.
