This case series describes four rare cases of optic nerve head capillary hemangiomas, including bilateral involvement and spontaneous regression. All patients maintained stable vision without progression to rubeosis iridis, retinal detachment, or blindness. No treatment was required. Retinal and optic nerve hemangiomas are often the earliest sign of von Hippel-Lindau (VHL) disease. Therefore, every patient with these findings should be evaluated for VHL. Appropriate imaging, genetic testing, and multidisciplinary lifelong surveillance are recommended. Ophthalmologists play a key role in early recognition and referral for multisystemic evaluation.
