Higher loop diuretic dosing after HF hospitalization is linked to worse clinical and patient-reported outcomes.
Why the study?
The TRANSFORM-HF trial found no difference in clinical outcomes between torsemide versus furosemide, motivating this analysis to assess the impact of diuretic dosing on primary and secondary clinical outcomes.
Does higher loop diuretic dosing improve clinical and patient-reported outcomes in patients following hospitalization for heart failure?

Does higher loop diuretic dosing improve clinical and patient-reported outcomes in patients following hospitalization for heart failure?
Higher loop diuretic dosing following heart failure hospitalization is associated with worse clinical and patient-reported outcomes, regardless of whether torsemide or furosemide is used.
Supports caution with high-dose loop diuretics after HF hospitalization; leaves open whether dose reduction improves outcomes.
AIMS: The TRANSFORM-HF trial found no difference in clinical outcomes between torsemide versus furosemide after hospitalization for heart failure. This analysis aimed to assess the impact of diuretic dosing on the primary and secondary clinical outcomes. METHODS AND Changes in KCCQ-CSS from baseline at 12 months was similar across dose groups for both drugs. CONCLUSION: Following hospitalization for heart failure, higher loop diuretic dosing was independently associated with worse clinical and patient-reported outcomes. The correlation between higher loop diuretic dose and outcomes was consistent, regardless of loop diuretic used.

Nouhravesh et al. (2024) conducted an RCT in Heart failure. Higher loop diuretic dosing (torsemide or furosemide) vs. Lower loop diuretic dosing was evaluated on Clinical and patient-reported outcomes (p=> 0.2). Following hospitalization for heart failure, higher loop diuretic dosing was independently associated with worse clinical and patient-reported outcomes, regardless of the loop diuretic used.
