Failing to terminate non-paroxysmal AF with ibutilide post-PVI is linked to ~99% higher arrhythmia recurrence.
Why the study?
The study aimed to evaluate how cardiac rhythm response to ibutilide infusion following pulmonary vein isolation relates to long-term atrial arrhythmia recurrence.
Does the cardiac rhythm response to ibutilide infusion after pulmonary vein isolation predict the recurrence of long-term atrial arrhythmias in patients with non-paroxysmal atrial fibrillation?

Does the cardiac rhythm response to ibutilide infusion after pulmonary vein isolation predict the recurrence of long-term atrial arrhythmias in patients with non-paroxysmal atrial fibrillation?
The response to ibutilide infusion after pulmonary vein isolation can predict long-term recurrence of atrial arrhythmias in patients with non-paroxysmal atrial fibrillation, aiding in risk stratification.

May aid post-PVI risk stratification in non-paroxysmal AF; leaves open prospective validation of ibutilide response.
PURPOSE: This study aimed to assess the relationship between the cardiac rhythm response to ibutilide infusion after pulmonary vein isolation and the recurrence of long-term atrial arrhythmias. METHODS: = 52). The primary endpoint was any documented recurrence of atrial arrhythmia lasting more than 30 seconds after the initial catheter ablation. CONCLUSIONS: In patients whose atrial fibrillation persisted after pulmonary vein isolation, the response to ibutilide administration could predict the recurrence of atrial arrhythmias after catheter ablation, which may be useful for risk stratification for recurrence of atrial fibrillation and individualised management of atrial fibrillation.

Wu et al. (2022) conducted a cohort in Non-paroxysmal atrial fibrillation (n=138). Ibutilide (responders) vs. Ibutilide non-responders was evaluated on Recurrence of atrial arrhythmia lasting more than 30 seconds (HR 1.994, 95% CI 1.117-3.561, p=0.020). In patients with non-paroxysmal atrial fibrillation, failure to terminate arrhythmia with ibutilide after pulmonary vein isolation was associated with a higher recurrence of atrial arrhythmias (HR 1.994).
