To determine the safety, including rates of autoimmune flares and immune-related adverse events, and efficacy of ipilimumab in patients with advanced melanoma and preexisting autoimmune disorders.
Retrospective review of N=30 patients with advanced melanoma and preexisting autoimmune disorders treated with ipilimumab across 9 academic tertiary referral centers between 2012 and 2015.
Preexisting conditions included rheumatoid arthritis (n=6), inflammatory bowel disease (n=6), psoriasis (n=5), systemic lupus erythematosus (n=2), multiple sclerosis (n=2), autoimmune thyroiditis (n=2), and other conditions (n=7), with 43% (13/30) receiving baseline immunosuppression.
Autoimmune disease exacerbations requiring systemic therapy occurred in 27% (8/30) of patients, with all flares managed using corticosteroids.
Grade 3 to 5 conventional immune-related adverse events occurred in 33% (10/30) of patients and resolved with corticosteroids or infliximab (n=2), alongside 1 treatment-related death from presumed colitis.
The objective response rate was 20% (6/30), including 1 durable complete response, while 50% (15/30) of patients experienced neither autoimmune flares nor conventional immune-related toxicities.
