Background: The orbit plays a crucial structural role in facial architecture, and bilateral orbital asymmetry substantially disrupts overall facial symmetry. Due to its anatomic prominence, the orbit is highly vulnerable to traumatic injury, and insufficient treatment or incomplete reconstruction can profoundly compromise long-term facial contour outcomes. Methods: This retrospective study enrolled subjects diagnosed with unilateral neglected orbital fractures at Shanghai Jiao Tong University Affiliated Ninth People’s Hospital between January 2025 and June 2025, excluding those with brain injuries, unstable vital signs, or trauma to other systemic locations. All subjects underwent surgical reconstruction performed by the same surgeon, with postoperative efficacy assessed using parameters such as zygomatic prominence height, frontal bone height, and degree of exophthalmos. Results: Six patients (6 eyes) with neglected orbital fractures were included in this study. Preoperative and postoperative height changes were statistically significant ( P <0.01). Preoperative heights were different on both sides ( P =0.631). Following the procedure, the height was nearly symmetrical between the operated side and the contralateral (healthy) side ( P <0.01). On the basis of our data, there was no change in exophthalmometry measurements on the affected side before and after surgery. Conclusion: This study presents a series of patients with bilateral orbital asymmetry resulting from congenital anomalies or acquired defects, in whom orbital volume augmentation was performed using ultra–high-molecular-weight polyethylene (SynPOR), with consistently favorable aesthetic and functional results achieved across all cases.
