3D mapping-guided conduction system pacing improves LV function and narrows QRS in a single patient.
Why the study?
Conduction system pacing for CRT requires high precision targeting narrow cardiac structures, but long fluoroscopy times remain a major limitation.
Does conduction system pacing guided by 3D electro-anatomical mapping improve clinical status and left ventricular function in a patient with non-ischemic dilated cardiomyopathy and left bundle branch block?

Does conduction system pacing guided by 3D electro-anatomical mapping improve clinical status and left ventricular function in a patient with non-ischemic dilated cardiomyopathy and left bundle branch block?
3D electro-anatomical mapping is a valuable tool to guide conduction system pacing, achieving narrow QRS and clinical improvement with low fluoroscopy exposure in complex anatomies.

May support 3D mapping-guided pacing in complex cases; hypothesis-generating and requires prospective validation.
Pestrea et al. (2023) conducted a case report in Non-ischemic dilated cardiomyopathy and left bundle branch block (n=1). Conduction system pacing guided by 3D electro-anatomical mapping was evaluated on Clinical status and left ventricular function. In a 60-year-old patient, conduction system pacing guided by 3D electro-anatomical mapping achieved a narrow QRS complex and significant improvement in clinical status and LV function at 3 months.
