Catheter ablation and implantable devices provide effective curative and symptomatic treatments for cardiac arrhythmias and heart failure, overcoming the limitations of antiarrhythmic drugs.
This review highlights the paradigm shift in clinical electrophysiology from pharmacological management to catheter ablation and implantable devices for the treatment of arrhythmias.
Knowledge about arrhythmia mechanisms is expanding rapidly, as a result of molecular biology, cellular electrophysiology and whole heart studies in animals and humans. Most clinically important arrhythmias result from reentry while others have a focal origin: both can be cured by catheter ablation. Antiarrhythmic drugs have disappointing efficacy, tolerability and safety, so attention has shifted to curative catheter ablation and use of implantable devices.
