Abstract

摘要: 卒中后抑郁(post-stroke depression,PSD)作为脑卒中后高发的重要神经精神并发症之一,不仅显著阻碍患者神经功能康复进程,更与预后病死率上升密切相关。传统西医治疗以5-羟色胺再摄取抑制剂(SSRIs)为主,但存在起效慢、不良反应多、耐药性等缺点。针刺疗法凭借其多靶点调控、低不良反应的优势,逐渐成为PSD治疗的研究热点。此文系统整合近年来国内外研究进展,从神经递质调控、抗炎抗氧化、神经可塑性及脑肠轴调节等多维度机制切入,结合临床疗效优化方案、多学科技术创新及国际研究差异,全面评述研究现状与挑战。当前各项研究正面临机制深度不足、临床证据等级偏低等问题。未来需强化国际合作,推动针刺从“经验医学”向“精准医学”转型,构建中西医融合的PSD综合管理体系。
