Bibliometric analysis maps MSC-derived exosomes' impact in asthma and COPD, indicating growth in research and collaborative efforts.
A BSTRACT Background: Mesenchymal stem cell (MSC)-derived exosomes have emerged as promising cell-free therapeutic agents for inflammatory respiratory diseases such as asthma and chronic obstructive pulmonary disease (COPD). Objective: This study aimed to map the global research landscape, major contributors, and thematic evolution of MSC-derived exosome research in asthma and COPD using bibliometric analysis. Materials and Methods: English-language articles and reviews published between 2020 and 2026 were retrieved from the Web of Science Core Collection. A total of 109 records were analyzed using Bibliometrix and Python-based visualization tools to evaluate publication trends, citation impact, productive countries, journals, authors, institutions, keywords, and thematic evolution. Results: Scientific production increased overall and reached its highest level in 2025, whereas average citation indicators declined over time. China led the field in publication output, total citations, and international collaboration, followed by Iran and the United States. Stem Cell Research and Therapy was the most productive journal ( n = 11). Keyword analysis identified “exosomes,” “extracellular vesicles,” and “asthma” as the most prominent themes, with increasing attention to immunomodulation and mechanistic pathways. Thematic evolution shifted from early mechanistic studies toward disease-oriented and translational research. Emerging topics such as apoptotic bodies and long-term therapeutic effects indicate expanding research frontiers. Conclusion: Research on MSC-derived exosomes in asthma and COPD is growing rapidly. Stronger collaboration, methodological standardization, and clinically oriented studies are needed to translate bibliometric momentum into therapeutic impact.
