LBBAP corrects iatrogenic bifascicular block and narrows QRS duration to ~125 ms.
Why the study?
RBBB correction by LBBAP has been attributed to longitudinal dissociation within the His bundle, which would not explain resolution of conduction block in the distal right bundle.
Does left bundle branch area pacing correct iatrogenic right bundle branch block located distal to the His bundle in a patient with cardiac amyloidosis?
Case Report (n=1)
No
Does left bundle branch area pacing correct iatrogenic right bundle branch block located distal to the His bundle in a patient with cardiac amyloidosis?
Absolute Event Rate: 125% vs 161%
LBBAP can correct iatrogenic right bundle branch block caused by lesions distal to the His bundle, likely due to engagement of a septal intramyocardial conduction network.
LBBAP may correct iatrogenic distal RBBB; leaves open generalizability and mechanism beyond this single case.
BACKGROUND: Correction of right bundle branch block (RBBB) by left bundle branch area pacing (LBBAP) has traditionally been attributed to capture within a longitudinally dissociated His bundle. Based on this theory, conduction block in the distal right bundle would not be expected to resolve. CASE SUMMARY: We describe a 90-year-old man with cardiac amyloidosis who underwent pacemaker implantation. During the procedure, advancement of the delivery catheter toward the right ventricular septum induced iatrogenic complete RBBB. Remarkably, pacing from this site corrected the block. The lead was then advanced >2 cm intraseptally, achieving successful LBBAP. Postoperatively, intrinsic conduction remained as RBBB, but unipolar pacing consistently corrected it. DISCUSSION: Correction of an iatrogenic RBBB by LBBAP suggests the involvement of a septal intramyocardial conduction network as a potential mechanism. TAKE-HOME MESSAGE: RBBB due to a lesion distal to the His bundle may be correctable with LBBAP.

Sumiyoshi et al. (2025) conducted a case report in Transthyretin cardiac amyloidosis with heart failure and atrial fibrillation requiring pacing (n=1). Left bundle branch area pacing (LBBAP) vs. Intrinsic conduction was evaluated on QRS duration and correction of bundle branch block. Left bundle branch area pacing successfully corrected an iatrogenic right bundle branch block and a left anterior fascicular block, narrowing the QRS duration to 125 ms.
