Does surgical aortic valve replacement reduce ascending aortic peak systolic energy loss and abnormal flow patterns assessed by 4D-flow MRI in a patient with severe aortic regurgitation caused by a quadricuspid aortic valve?
4D-flow MRI can visualize abnormal ascending aortic flow and quantify reductions in peak systolic energy loss following surgical aortic valve replacement in a patient with a quadricuspid aortic valve.

4D-flow MRI quantifies post-SAVR energy loss reduction in quadricuspid aortic valve; hypothesis-generating and should not yet change practice.
IMAGES: Four-dimensional-flow magnetic resonance imaging demonstrated prominent helical and vortical flow in the ascending aorta of a patient with severe aortic regurgitation caused by a quadricuspid aortic valve. CASE SUMMARY: Peak systolic energy loss in the ascending aorta was 68.3 mW and decreased to 15.7 mW after surgical aortic valve replacement, accompanied by marked reduction in abnormal flow patterns. TAKE-HOME MESSAGE: This case demonstrates the ability of 4-dimensional-flow magnetic resonance imaging to visualize abnormal ascending aortic flow and quantify associated energy loss beyond conventional valvular assessment.

Inno et al. (2026) conducted a case report in Severe aortic regurgitation caused by a quadricuspid aortic valve (n=1). Surgical aortic valve replacement assessed by 4D-flow MRI vs. Preoperative baseline was evaluated on Peak systolic energy loss in the ascending aorta. Surgical aortic valve replacement reduced ascending aortic peak systolic energy loss on 4D-flow MRI from 68.3 mW to 15.7 mW in a patient with a quadricuspid aortic valve.
