Downstream cangrelor shows no benefit over upstream ticagrelor for in-hospital MACE during STEMI primary PCI.
Why the study?
Intravenous cangrelor provides an alternative in STEMI referred to PCI where oral absorption can be hindered, prompting a comparison between downstream cangrelor and upstream ticagrelor.
Does downstream cangrelor reduce in-hospital MACE compared to upstream ticagrelor in P2Y12 inhibitor-naïve STEMI patients undergoing primary PCI?

Does downstream cangrelor reduce in-hospital MACE compared to upstream ticagrelor in P2Y12 inhibitor-naïve STEMI patients undergoing primary PCI?
Downstream cangrelor and upstream ticagrelor show comparable in-hospital ischemic and bleeding outcomes in P2Y12 inhibitor-naïve STEMI patients undergoing primary PCI.

Greco et al. (2024) conducted a cohort in ST-segment elevation myocardial infarction (STEMI) (n=761). Downstream cangrelor vs. Upstream ticagrelor was evaluated on Composite of in-hospital major adverse cardiovascular events (MACE) (OR 1.30, 95% CI 0.79-2.17, p=0.308). Downstream cangrelor showed no significant difference in in-hospital MACE compared to upstream ticagrelor in STEMI patients undergoing primary PCI (OR 1.30; 95% CI 0.79-2.17; P=0.308).
