To systematically quantify the association between parental smoking and acute lower respiratory illness during the first three years of life.
Systematic search of Embase and Medline databases through April 1997 identifying 50 relevant publications from 692 screened articles.
Quantitative synthesis of 38 community- and hospital-based studies using random-effects modeling to calculate pooled odds ratios.
Smoking by either parent was significantly associated with acute lower respiratory illness (pooled OR 1.57, 95% CI 1.42 to 1.74), with risk highest for maternal smoking (pooled OR 1.72, 95% CI 1.55 to 1.91).
Smoking by other household members in homes where mothers did not smoke also elevated illness risk (pooled OR 1.29, 95% CI 1.16 to 1.44), with evidence of a dose-response relationship across studies.
