Abstract Spontaneous mediastinal emphysema is rare and is usually associated with a sudden increase in alveolar pressure. The most common presenting symptom is acute chest pain. We report the case of a young man who presented to the emergency department with abdominal pain, which was ultimately diagnosed as spontaneous pneumomediastinum associated with cervical emphysema.
