Pulsed-field ablation shows feasibility and success for treating AF in a Fontan palliation case.
Why the study?
Pulsed-field ablation is effective for AF and promising in congenital heart disease, but its use in Fontan patients had not been reported.
Is pulsed-field ablation feasible and successful for treating atrial fibrillation in a patient with Fontan palliation?
Case Report (n=1)
Is pulsed-field ablation feasible and successful for treating atrial fibrillation in a patient with Fontan palliation?
Pulsed-field ablation appears feasible and successful for treating atrial fibrillation in patients with complex congenital heart disease such as Fontan palliation.
May inform ablation strategy in Fontan patients; leaves open prospective validation of safety and efficacy.
INTRODUCTION: Pulsed-field ablation (PFA) is a novel and effective modality for ablation in patients with atrial fibrillation (AF). Available case reports and small series showed promising results in patients with congenital heart disease. However, PFA ablation in Fontan patients has not been reported yet. METHODS AND RESULTS: We present a 35-year-old patient with a history of Fontan palliation for hypoplastic left heart who underwent successful ablation for AF with PFA. We describe procedure characteristics and illustrate the potential benefits of PFA in this patient population. CONCLUSION: We present a case of PFA for AF in Fontan palliation. PFA was feasible and successful in our case. Long-term outcomes are yet to be determined.

Khalil et al. (2025) conducted a case report in Atrial fibrillation in Fontan palliation (n=1). Pulsed-field ablation (PFA) was evaluated on Feasibility and success of ablation. Pulsed-field ablation was feasible and successful for treating atrial fibrillation in a 35-year-old patient with a history of Fontan palliation.
