Successful percutaneous balloon mitral valvotomy is linked to improved peak atrial longitudinal strain.
Why the study?
Relieving mitral obstruction improves left atrial mechanics and structural remodelling, but this is not typically used as a measure of successful balloon mitral valvotomy in clinical practice.
Does percutaneous balloon mitral valvotomy improve left atrial strain in patients with severe mitral stenosis?
Observational

Does percutaneous balloon mitral valvotomy improve left atrial strain in patients with severe mitral stenosis?
Peak atrial longitudinal strain significantly improves after successful balloon mitral valvotomy and may serve as a supportive indicator of procedural success alongside established markers.

May support strain as adjunct success marker post-valvotomy; leaves open causal benefit and need for randomized trials.
BACKGROUND: Severe mitral stenosis (MS) leads to morphological and functional changes in the left atrium (LA) causing dysfunction. Relieving mitral obstruction improves the LA mechanics and structural remodelling. However, this is not taken as a measure of successful Ballon Mitral Valvotomy (BMV) in clinical practice. METHODS: We studied the relationship between LA strain and severe MS and the short-term effect of BMV on LA mechanics in a prospective observational single-centre study. CONCLUSION: Peak atrial longitudinal strain shows a significant improvement following a successful BMV. This increase in values reflects an improvement in clinical status and function class in follow-up. While PALS offers promise, it has limitations. Therefore, it seems reasonable to consider PALS as a supportive indicator alongside established markers like MV area and pressure gradient for assessing procedural success.

Pant et al. (2025) conducted an observational in Severe mitral stenosis. Percutaneous balloon mitral valvotomy (BMV) was evaluated on Peak atrial longitudinal strain (PALS) (p=0.038). Peak atrial longitudinal strain showed a significant improvement following successful percutaneous balloon mitral valvotomy (p=0.038), reflecting improved clinical status.
