This randomized trial identifies chronic pelvic pain as an independent reproductive suppressor, suggesting a need for new treatment approaches.
Chronic pelvic pain (CPP) affects 10–25% of women of reproductive age and is documented in up to 71% of patients with unexplained infertility or recurrent implantation failure. Despite this prevalence, CPP is consistently treated as a symptom of underlying organic pathology — never as an autonomous pathological agent. This creates a critical clinical blind spot: assisted reproductive technology (ART) cycles are initiated in a neurobiological environment actively hostile to implantation, producing unexplained failure, escalating patient burden, and significant socioeconomic cost. Objective. To introduce, formally define, and systematise the original clinical doctrine of Neuroreproductive Pain© (NRP©) — establishing chronic pelvic pain as a primary, measurable, and independently acting suppressor of reproductive function, and presenting its validated staging instrument (Hudziak NRP-Index®) and stepped-care treatment framework (6D-Interception Algorithm®). Evidence Base. The NRP© doctrine integrates converging evidence from neuroimmunology (NGF/Substance P pathways), autonomic medicine (HRV/RMSSD as reproductive readiness proxy), osteopathic neuro-lymphatic mapping (Chapman–Goodheart reflex points), reproductive endocrinology (HPO axis disruption by CRH/beta-endorphin), and clinical outcomes from the author's specialist practice at Viktoriya Hudziak Reproductive Clinic©, Ukraine. Core Finding. Chronic pelvic pain, through ten interrelated pathophysiological components, induces a systemic reproductive block via neurogenic inflammation, central sensitization, and autonomic distress. The cascade terminates in gamete epigenetic damage and neuro-immune implantation failure — mechanisms that operate independently of, and in addition to, any structural reproductive pathology. Conclusions. NRP© is a distinct, stageable, and therapeutically interceptable clinical entity. Its recognition transforms reproductive medicine from organ-based symptom management to active Neuroreproductive Balance Engineering©.
