The paper discusses response rates to a health survey questionnaire, rates of return of medical record release consent forms and response rates to re‐interview and follow‐up medical examination. The telephone and personal strategies appear to yield comparable rates which are higher than those of the mail strategy, except in the case of return of medical record release consent forms where the situation is reversed.
