Fluoroless 3D mapping-guided ablation eliminates Purkinje-related ventricular tachycardia with no recurrence at 1 year.
Case Report (n=1)
High-density 3D electroanatomic mapping can precisely visualize the reentrant circuit of left fascicular ventricular tachycardia within the Purkinje network, enabling successful and safe fluoroless ablation.
Demonstrates feasibility of fluoroless 3D mapping-guided ablation in left fascicular VT; leaves open prospective validation in larger cohorts.
Left Fascicular Ventricular tachycardia (LFVT), affects young adults (15–40 years) with structurally normal hearts. In 90% of cases LFVT originates from the left posterior fascicle and has a right-bundle brunch block (RBBB) pattern with left axis deviation and in rare cases with right axis deviation. LVFT is often responsive to calcium channel blockers administration 1-3 . In some cases LFVT may be incessant and may cause a reversible tachycardia-related cardiomyopathy.

Scaglione et al. (2023) conducted a case report in Left Fascicular Ventricular Tachycardia (LFVT) (n=1). 3D electroanatomic mapping-guided fluoroless transcatheter ablation was evaluated on Tachycardia inducibility and clinical recurrence. Three-dimensional electroanatomic mapping successfully visualized the reentrant circuit in the Purkinje network, guiding effective fluoroless radiofrequency ablation with no arrhythmia recurrence at 1 year.
