Does surgical cryoablation guided by computational ECG mapping reduce PVC burden in a patient with an anomalous RCA?
This case demonstrates the feasibility of using computational ECG mapping to guide successful surgical cryoablation of PVCs originating near an anomalous coronary artery.

May support ECG-guided surgical cryoablation for PVCs in anomalous RCA; hypothesis-generating and requires prospective validation.
Introduction: Premature ventricular contractions (PVC) and anomalous coronary arteries have not been previously associated. Methods: 40-year-old female presented with symptomatic PVCs. Pre-ablation imaging revealed an anomalous right coronary artery (RCA) originating from the left coronary cusp with an interarterial, intramural course. Novel ECG mapping localized PVCs to the septal right ventricular outflow tract (RVOT) near the anomalous RCA. Results: Surgical PVC cryoablation at the endocardial RVOT was performed, followed by unroofing surgery, with <1% PVCs at 2 years. Conclusion : This is the first report associating PVCs to the site of an anomalous RCA. Surgical cryoablation during unroofing surgery was guided by novel ECG mapping.

Jackson et al. (2023) conducted a case report in Symptomatic premature ventricular contractions and anomalous right coronary artery (n=1). Surgical PVC cryoablation guided by computational ECG mapping and unroofing surgery was evaluated on PVC burden. Surgical PVC cryoablation guided by novel ECG mapping during unroofing surgery successfully treated a patient with an anomalous right coronary artery, resulting in <1% PVCs at 2 years.
