Does adjunctive thrombectomy improve outcomes in patients with acute myocardial infarction?
The authors clarify that a subanalysis limited to trials requiring angiographically visible thrombus would yield extremely wide credible intervals due to only 5 of 21 trials having this requirement.
Response:Angiographically visible thrombus was required in only 5 of the 21 trials 1-5 ; therefore, a subanalysis limited to these trials would have yielded results with extremely wide credible intervals. We agree with Drs Lozano, Avanzas, and Rondan that there remains some confusion regarding whether thrombus removal should be performed in all comers or only in patients with obvious thrombus at coronary angiography.Thrombectomy is unlikely to work by "intention to treat"; thrombus has to be removed.
