SUMMARY The glycoprotein of Tamm and Horsfall is a normal urinary constituent synthesized and secreted by the epithelial cells of the ascending limb of Henle and the macula densa segment of the distal convoluted tubule. Increased excretion has been observed with acute renal tubular damage. Seven acute rejection episodes were observed in five patients receiving human renal allografts. Abnormally high excretion was observed between 4 and 14 days prior to the time that rejection could be diagnosed by clinical criteria.
