Objective: To evaluate the frequency of self-reported symptoms under real-life conditions by patients performing home blood pressure monitoring (HBPM) using the Hy-Result web application (app). Design and method: Hy-Result® is a validated app endorsed by the French Society of Hypertension (SFHTA) an Belgium Hypertension Committee (BHC), which: i) helps patients measuring BP according to a standardized protocol; ii) automatically interprets the readings; iii) allows measurements to be shared via a PDF report. Each report is generated by a patient measuring BP (morning and evening, 3 measurements per session) for 3 to 7 consecutive days. Data were collected between September 2024 and February 2025. The analyses are based on whether or not antihypertensive medication is taken, self-reported symptoms (headaches, dizziness, pain, etc.) classified into three categories: i) no symptoms, ii) mild symptoms, and iii) severe symptoms, BP levels, and frequency of app use. Results: A total of 20,900 reports were analyzed. Population: average age 65 years; 67% (n=14,052) receiving antihypertensive treatment; 24% (n=5,861) with comorbidities (diabetes, chronic kidney disease, stroke); 58% (n=12,190) male. Among treated patients, 66% (n=92,227) of reports showed BP >130/80; among untreated patients, 43% (n=2,977) of reports confirmed hypertension diagnosis (BP >135/85). Overall, 20% (n=4,175) of reports indicated the presence of symptoms (moderate 17% (n=247), severe 3% (n=607). Significant correlations were observed between symptom frequency and several variables: Younger patients were more symptomatic (p 160/110 mmHg) were associated with more frequent symptoms (p < 0.001). Conclusions: Observing that 20% of patients performing HBPM in real life declare symptoms, and that patients undergoing treatment are less symptomatic (p < 0.001) in the large Hy-Result cohort, presents original data for which there are few comparisons in the literature.
