OBJECTIVE: This study aimed to explore the epidemiological characteristics of injury-related mortality among individuals aged 1-24 years in Lanzhou, China, from 2014 to 2023. METHODS: The study analysed injury-related mortality data among individuals aged 1-24 years in Lanzhou (2014-2023), calculating crude and standardised mortality rates, along with injury proportions. The Joinpoint Regression was used to determine the annual percentage change (APC) and average APC (AAPC) to identify trends. Incidence ratios (IR) assessed temporal differences, while spatial autocorrelation used Moran's I and Local Indicators of Spatial Autocorrelation clustering. Poisson regression evaluated effects of month, age, sex and region. RESULTS: The age-standardised mortality rate was 7.7 per 100 000, the rate decreased in males (AAPC: -8.2%, 95% CI -13% to -2.8%) and in children aged 1-4 years (AAPC: -10%, 95% CI -16% to -4.2%) and 5-9 years (AAPC: -16%, 95% CI -27% to -3.8%). Males had higher mortality than females (IR=1.97, 95% CI 1.70 to 2.31). Road injuries were the leading cause of death across all age groups, with the highest mortality rates among adolescents aged 15-19 years, showing elevated mortality risks during summer and weekends. Self-harm was predominant among individuals aged 10-24 years, showing a distinct bimodal distribution with mortality peaks in February and June. Falls contributed to mortality in all age groups, with higher risks during summer and holiday periods. Spatially, Honggu and Chengguan have higher injury-related mortality rates, while Anning has the lowest. CONCLUSION: This study identified sex, age and spatiotemporal variations in injury-related mortality among children, adolescents and young adults in Lanzhou.
