Background Neck and/or shoulder pain (NSP) is widely reported among school-aged children, with documented detrimental effects on their physical health and psychological status. This study aims to identify the prevalence and associated factors of NSP among primary school children in Shijiazhuang City, Hebei Province, China. Methods In this cross-sectional study, 362 primary school children aged 10–12 years from Shijiazhuang city, northern China, were recruited using a multistage cluster random sampling method. Participants completed the fundamental motor skills test and questionnaires assessing socio-demographics, physical activity, psychological distress, quality of life, and NSP. Data were analyzed using univariable and multivariable logistic regression. Results The one-month prevalence of NSP was 33.7%. Multivariable logistic regression showed that female students had a significantly higher odds of NSP compared with males adjusted odds ratio [AOR = 1.61, 95% confidence interval CI: 1.01–2.56, P = 0.047]. In addition, higher psychological symptom scores were independently associated with an increased odds of NSP (AOR = 1.04, 95% CI: 1.01–1.09, P = 0.036). Conversely, moderate physical activity (AOR = 0.51, 95% CI: 0.29–0.89, P = 0.018) and higher quality of life scores (AOR=0.98, 95% CI: 0.95–0.99, P = 0.026) were linked to lower odds of NSP. Conclusion NSP is prevalent among Chinese children. Moderate-intensity physical activity, quality of life, and psychological status were associated with NSP. These findings highlight the importance of implementing early interventions that promote physical activity and psychological well-being to mitigate NSP in children.
