Key result
Although plasma levels of big endothelin, PIIINP, and MMP-2 were associated with early HFNEF, none of these biomarkers contributed independently to noninvasive HFNEF diagnostics.
Why the study?
Do markers of collagen turnover or other biomarkers contribute to the diagnosis of heart failure with normal ejection fraction in patients with exertional dyspnea?
Observational (n=111)
No
Do markers of collagen turnover or other biomarkers contribute to the diagnosis of heart failure with normal ejection fraction in patients with exertional dyspnea?
While plasma levels of big endothelin, PIIINP, and MMP-2 are elevated in early HFNEF, they do not provide independent diagnostic value beyond current guideline-recommended echocardiographic and NT-proBNP assessments.
Answers are grounded in this paper and cite their sources.
Biomarkers should not yet inform noninvasive HFNEF diagnosis; leaves open prospective validation beyond echocardiography and NT-proBNP.

Meluzı́n et al. (2013) conducted an observational in Heart failure with normal ejection fraction (HFNEF) (n=111). Plasma biomarkers (big endothelin, PIIINP, MMP-2) vs. Patients without HFNEF and healthy controls was evaluated on Independent prediction of HFNEF in multivariate logistic regression (Big endothelin) (OR 6.12, 95% CI 0.26-142.96, p=0.260). Although plasma levels of big endothelin, PIIINP, and MMP-2 were associated with early HFNEF, none of these biomarkers contributed independently to noninvasive HFNEF diagnostics.
