Abstract Introduction Wearable technology now offers a real-world insight into the characteristics of nocturnal penile tumescence (NPT). RigiScan (RS) and FirmTech TechRing (TR) are currently the only available penile wearable devices capable of measuring erection duration, frequency and rigidity. Objective We aimed to compare the diagnostic accuracy and concordance between the TR and RS in the measurement of nocturnal erectile activity. Methods We conducted a non inferiority study from April to October 2025. We included adult men who presented for ED. Each patient underwent simultaneous NPT recordings with both TR and RS on the same night under standardized laboratory conditions. TR was fitted at the penile base, while RS was applied according to the manufacturer’s guidelines at two locations on the penile shaft. The recorded data (erectile rigidity, frequency, duration) were analyzed and compared. Results 27 recordings were analyzed. RS recorded a median of 4 events (IQR 2, 5) compared to a median of 4 events (3, 5) using TR. RS reported a median of 53 minutes (22.5, 99) of erectile activity, TR 66 minutes (30, 98.5), which was statistically different (p 0.001). Graphical analysis indicated that RS tended to under-report event duration, whereas TR more closely reflected the actual recorded time. In a subgroup analysis by rigidity percentile, for the ≥80% rigidity range, both RS and TR showed significantly different mean durations of 11.6 minutes (22.2) and 30.1 minutes (28.1), respectively. Spearman correlation analysis demonstrated significant concordance between the two devices in detecting high-rigidity events (p=0.03). Conclusions Firmtech’s TR demonstrated greater sensitivity in both graphical representation and recorded data compared to RS, particularly in overall event duration. These differences likely reflect distinct device algorithms and sensitivity thresholds. FirmTech also showed high concordance with RigiScan in detecting episodes of high rigidity, supporting its validity in measuring maximal erectile firmness.
