Why the study?
The registry aimed to evaluate characteristics and treatment strategies in patients with unstable angina or non-Q wave myocardial infarction treated with the glycoprotein IIb/IIIa antagonist tirofiban in Switzerland.
What are the characteristics and treatment strategies in patients with unstable angina or non-Q wave myocardial infarction treated with tirofiban in Switzerland?
Observational (n=171)
Yes
What are the characteristics and treatment strategies in patients with unstable angina or non-Q wave myocardial infarction treated with tirofiban in Switzerland?
In a Swiss registry of ACS patients treated with tirofiban, medical stabilization was rapidly achieved, but invasive evaluation was performed in a low percentage of patients, particularly women and older individuals.
May support conservative ACS management after tirofiban; leaves open optimal catheterization thresholds by age and sex.
The acute coronary syndrome in Switzerland: Insights from the multicenter MACS (management in acute coronary syndrome in Switzerland) registry. The purpose of the prospective registry “Management of Acute Coronary Syndrome in Switzerland” (MACS) was to evaluate characteristics and treatment strategies in patients with unstable angina (UA) or non-Q wave myocardial infarction (NQMI) who where treated with the glycoprotein 11b/IIIa antagonist Tirofiban in Switzerland. In the period from July 1998 to April 1999 we evaluated 171 patients (23% women) admitted to 25 Swiss hospitals. There was no significant age difference between men and women (median 69 vs. 64 yrs.). The diagnosis of UA was more frequent (71%) than NQMI. Differences were observed in coronary risk profiles with a higher incidence of hypercholesterolemia, diabetes and systemic hypertension in women and patients over 64 years, whereas the incidence of smoking was significantly higher in men and younger patients. Coronary artery disease was known in 80% (n = 136) of the patients enrolled. The time delay between onset of symptoms and hospitalization was 6 hours (median) and was significantly shorter in patients over 64 years of age (5.3 vs. 7.8, p = 0,003). Medical treatment in addition to Tirofiban included aspirin (95%), nitrates (85%), heparin (97%) and betablocker (67%). Patients could be stabilized by medical management in 80% within 6 hours. The rate of cardiac catheterization was 32% and coronary angioplasty 19% (76% after catheterization). Coronary angiography was performed less often in women (26% women vs. 34% men) as well as coronary angioplasty (10% women vs. 22% men). 7% of men underwent bypass operation. In patients over 64 years of age coronary catheterization was performed significantly less than in younger patients (23% vs. 42%, p = 0,009) with revascularization following in 90%. The median length of hospitalization was 7 days and was significantly longer in patients over 64 years (5,9 vs. 7,9, p = 0,047). The enrolled patients with acute coronary syndrome showed a relatively long pre-hospitalization time. After addmission rapid stabilization was achieved by medical treatment. Cardiac catheterization rate was especially low in patients over 64 years of age and in women. Rate of cardiac interventions was high after catheterization especially in patients over 64 years of age.
